Apply for a U.S. passport
The Clerk of Superior Court accepts passport applications by appointment. Bring the completed DS-11, proof of citizenship, a photo, and identification.
How it works
- Complete passport application form DS-11 (do not sign it in advance).
- Call 912-653-3871 to schedule an appointment.
- Appear in person; children under 14 need both parents present to sign.
What you need
- Completed form DS-11.
- Certified birth certificate, previous passport, or naturalization documents (children under 14: birth certificate and any previous passport).
- One 2" x 2" passport photo, color preferred.
- Photo identification, driver’s license preferred.
